Oscar Unlocks Program Terms and Conditions
The Oscar Unlocks Reward Program is a member-focused program that provides access to perks and discounts, while offering members the opportunity to earn points by completing qualified health-related or administrative activities. Members can redeem their earned points for a prepaid debit card or apply them directly toward their health insurance premiums, if applicable.
1. General Program Eligibility
Participation in the Oscar Unlocks Rewards Program is available to eligible active Oscar members, including subscribers and covered dependents age 18 and older. Participation does not guarantee eligibility for every reward. Reward availability and eligibility may vary based on the member’s plan, state, qualifying condition or medication, and completion of the applicable qualifying activity.
Participation is voluntary and at no additional cost to the member. Choosing not to participate does not affect a member’s eligibility for health coverage or covered benefits. Eligible members do not need to separately enroll in or activate Oscar Unlocks to begin earning eligible rewards. Some digital activities, program perks, and self-service features require an active Oscar account. Members without an Oscar account may contact the Oscar Care Team for assistance.
2. Reward Activities, Values and Limits
The Oscar Unlocks Rewards Program includes qualifying activities with an assigned reward value. Completing an activity more than once does not generate additional rewards, unless the activity rules expressly allow it. Reward values, qualifying activities, eligibility criteria, annual limits, and other Program requirements may vary by member, plan, and state. The maximum value a member may earn is subject to the applicable Program rules, policy, and any state-specific reward or incentive limits. Limits apply to rewards redeemed during the Plan Year. Oscar may add, remove, replace, or modify qualifying activities, reward values, eligibility criteria, vendors, redemption options, and other Program offerings during the plan year, subject to applicable Program terms and law.
Rewards may have tax consequences. Members are responsible for any applicable taxes associated with rewards they receive.
3. Earning and Verification
Reward points become available after Oscar verifies completion of the applicable qualifying activity and eligibility requirements. Verification may depend on administrative records, submitted documentation, or processed medical or pharmacy claims. Submission or payment of a claim does not by itself establish reward eligibility. Posting timing of rewards varies by activity and depends on when Oscar receives the information needed for verification.
Oscar may correct, withhold, or reverse points credited in error or associated with duplicate, reversed, fraudulent, or otherwise ineligible activity, as permitted by applicable Program terms and law. Members may contact the Oscar Care Team to request review of a missing, withheld, or reversed reward
4. Redemption
For eligible point-based rewards, points may be redeemed for a prepaid debit card or, where available, applied toward an Oscar premium. Premium redemption is not available in New York or New Jersey. Certain rewards may use a different fulfillment method. A member must have eligible active coverage in order to redeem rewards. Members may redeem available points through the Oscar Unlocks portal or by contacting the Oscar Care Team for assistance. Completed redemptions are final and cannot be reversed or converted back into points, except to correct an error or as required by law. Reward redemptions may be subject to processing delays and may not appear immediately in a member’s account.
Where premium redemption is available, the credit is applied to the next premium amount due as indicated by Oscar. A redemption request does not reduce the premium amount due until the credit is actually posted. Applying reward points toward a premium does not relieve the subscriber of responsibility for any remaining premium balance. Any remaining amount must be paid by the applicable due date and remains subject to the plan’s payment and grace-period rules.
5. Expiration and Loss of Eligibility
Unused reward points do not expire while a member remains continuously enrolled in an eligible Oscar plan. Unused reward points are forfeited when Oscar coverage terminates, except as required by law or expressly provided for an eligible coverage transition or reinstatement. Funds loaded to an Oscar Unlocks prepaid debit card expire upon disenrollment from the applicable Oscar plan. Treatment of card balances following changes in eligibility or reinstatement of coverage is governed by the applicable Program and Cardholder Agreement terms.
6. Prepaid Debit Card
The Oscar Unlocks prepaid debit card is issued as a reward at no cost to the member for completing a qualifying activity. The card is issued by Sutton Bank and administered by Incomm. Use of the card is governed by the applicable Cardholder Agreement, available at cardholderagreement.mybenefitscenter.com.
The card may be used at permitted merchants for permitted purchases, subject to the Cardholder Agreement and applicable merchant and purchase restrictions. Reward funds may not be used for certain purchases, including alcohol, tobacco, or firearms. Additional restrictions may apply. See the Cardholder Agreement at cardholderagreement.mybenefitscenter.com for complete card terms and restrictions.
7. Third-Party Offerings
Third-party discounts, memberships, marketplaces, and other offerings available through Oscar Unlocks are not covered services under the Oscar health plan unless expressly stated otherwise. Availability and eligibility may vary, and these offerings may be changed or discontinued. Third-party vendors are responsible for the products and services they provide and may impose their own terms, eligibility requirements, and restrictions.
8. Vision Discount Program
Vision discount-program availability varies by plan and state and is separate from vision benefits under the Oscar health plan. Members enrolled in certain plans with comprehensive vision coverage may not be eligible for the vision discount program.
9. Relationship to Health Coverage
Oscar Unlocks rewards do not change the terms of a member’s health insurance coverage or determine whether a healthcare service, treatment, prescription, provider, or other item is covered. Coverage, cost sharing, network requirements, exclusions, and other health-plan terms are governed by the applicable Evidence of Coverage and Schedule of Benefits.
10. Program Terms
These terms govern Program eligibility, earning, and redemption. The Cardholder Agreement governs card use and card-transaction rights.
Oscar may modify or discontinue the Oscar Unlocks Rewards Program as permitted by these terms and applicable law. Changes to the Program will not reduce funds already loaded to a prepaid debit card, except as otherwise permitted under the applicable Cardholder Agreement or law. Additional terms, restrictions, and exclusions may apply.
