Oscar requires updated rosters from providers or delegated entities to maintain accurate information in our online directory.

We encourage you to use one of our templates or update your current format to match our template. Please submit all rosters to rosters@hioscar.com.

Every 120 days, you or your credentialing delegate must review, update, and attest to the provider information in CAQH and changes must be reported at least 30 days before they take effect.

You may access the most current information we have for you at www.hioscar.com/search.

For providers credentialed through Oscar, submit demographic changes via your next roster file, the Provider Portal, or by calling Provider Services at (855) 672-2755, Option 4.

For providers who have a delegated credentialing agreement, changes should be submitted in the delegated entity’s next roster submission.

Ensure accuracy for:

• CAQH ID
• TIN and NPI
• PCP status
• Accepting new patients status
• Practice location addresses
• Office phone numbers and emails
• Group and facility affiliations
• Specialties
• Licenses
• Languages spoken
• Ages served
• Office hours
• ADA accessibility
• Telehealth services

Required:

• Only one file should be sent with all of the requested updates and data elements reflected on it.
• The singular file sent must either be a full file or changes (delta) file; not both.
• All roster updates must be sent in the singular file.
• Format of regular update files, sent on either a weekly or monthly basis, should be consistent. Any changes to the format, such as the addition, removal, or reordering of a column, should be communicated beforehand.

Preferred:

• File submission in .CSV or .XLSX format.
• If sending additions, terminations, and/or changes, these should all be in separate tabs.
• Multiple providers and office locations may be included on the same tab. Each service address should be designated by its own row.
• One roster per email. If multiple rosters need to be sent, please send them in individual emails.

Not accepted:

• Sending multiple files with data elements on different files and/or in different formats.
• Sending a Word document, PDF, or screenshots.
• Using multiple delimiters in the file.
• Sending regular updates in inconsistent formats.
• Merged cells in the file.
• Hidden columns and rows.
• Sending updates solely in the body of the email.
