Telemedicine is the practice of medicine using electronic communications technologies. Telemedicine consultation services are provided by Oscar network providers: Oscar Medical Group, P.A., and Teladoc Physicians, P.A. When you use these telemedicine consultation services, you will enter into a provider-patient relationship and be treated by a provider who is licensed in your state. The providers, in exercising their professional judgment, reserve the right to deny care for any reason, including instances where provision of care would be medically or ethically inappropriate.Getting treatment through a telemedicine consultation offers certain expected benefits and has certain potential risks. Expected benefits include easier and quicker access to medical care, and the ability to access care from many locations. For example, you can initiate a remote examination from your home rather than having to wait to schedule one. Potential risks include the remote physician not being able to fully resolve your issue over the phone, and lack of physician access to your full medical records, which may, in rare instances, result in allergic reactions or other adverse drug interactions. You have other options for care. You can obtain care in a traditional in-person care setting, such as seeing your Primary Care Physician or visiting an urgent care center. By agreeing to have a telemedicine consultation, you acknowledge that you have reviewed these benefits, risks, and alternatives, and you agree that you consent to a telemedicine consultation. You have the right to withdraw your consent at any time by ending this telemedicine consultation session. By agreeing to have a telemedicine consultation, you acknowledge you have received and read the Notice of Privacy Practices for Oscar’s partners, Oscar Medical Group, P.A., and you have received and read the Notice of Privacy Practices for Oscar’s partner Teladoc Physicians, P.A. You further authorize Oscar and its partners, Oscar Medical Group, P.A. and Teladoc Physicians, P.A., to release your confidential information to each other and consulting providers. You have read and understand Oscar’s Terms & Conditions, Privacy Policy and Notice of Privacy Practices and Teladoc’s Terms & Conditions and Privacy Policy. You agree to receive emails and push notifications (if enabled) about your consultation, including the provider’s name and consultation time.(A) The availability of receiving the service on an in-person basis or via telehealth, if available, from the enrollee’s primary care provider, treating specialist, or from another contracting individual health professional, contracting clinic, or contracting health facility consistent with the service and existing timeliness and geographic access standards in Sections 1367 and 1367.03 and regulations promulgated thereunder. (B) If the enrollee has coverage for out-of-network benefits, a reminder of the availability of receiving the service either via telehealth or on an in-person basis using the enrollee’s out-of-network benefits, and the cost sharing obligation for out-of-network benefits compared to in-network benefits and balance billing protections for services received from contracted providers. (2) After being notified pursuant to paragraph (1), the enrollee chooses to receive the service via telehealth through a third-party corporate telehealth provider. (3) The enrollee consents to the service consistent with Section 2290.5 of the Business and Professions Code. (4) If the enrollee is currently receiving specialty telehealth services for a mental or behavioral health condition, the enrollee is given the option of continuing to receive that service with the contracting individual health professional, a contracting clinic, or a contracting health facility.California members. I’ve reviewed and agreed to this state-specific consent when receiving virtual care services:I understand I have the right to access my medical records. Additionally, a summary of any services I’ve received from a third-party virtual care provider will be shared with my primary care provider — unless I request otherwise. I’ve also been informed that all services received through the third-party virtual care provider are available at in-network cost-sharing, and the cost-sharing will count towards my plan’s applicable deductible or out-of-pocket maximum.(A) The availability of receiving the service on an in-person basis or via telehealth, if available, from the enrollee’s primary care provider, treating specialist, or from another contracting individual health professional, contracting clinic, or contracting health facility consistent with the service and existing timeliness and geographic access standards in Sections 1367 and 1367.03 and regulations promulgated thereunder.
